New Case Report: Use of Respiratory Dialysis to Enable Lung Protective Ventilation in a Patient with Moderate ARDS due to Crush Syndrome from a Mining Accident

Smiechowicz User Report Thumb smA 41 year old male copper mine worker who developed moderate acute respiratory distress syndrome (ARDS) after being crushed and buried by falling stones in a mine accident. The patient became septic and difficult to ventilate in the days following amputation of his severely crushed left leg. He was transferred from a regional hospital to Wroclaw University Hospital for ECMO consideration. However, the primary difficulty in providing respiratory support was not oxygenation but rather carbon dioxide (CO2) removal. Instead of ECMO, the patient was treated with low-flow respiratory dialysis using the Hemolung RAS. With flows of 400-500 mL/min through a single, 15.5 French venous cannula, the Hemolung was successfully utilized to enable significant reduction of the high peak inspiratory pressures needed to normalize the patients pH and carbon dioxide tension and prevent ventilator-induced lung injury. The New Hemolung Registry

Registry Form Thumb smWe recently revised our registry form, aligning it with the ELSO Registry data collection and dramatically reducing the time required to enter data. We encourage all Hemolung RAS users to consider contributing their cases to the registry. Participants will be provided access to the data in forthcoming reports.

Recent Report in Critical Care Medicine

A recent report in Critical Care Medicine title, “Extracorporeal CO2 Removal in Hypercapnic Patients at Risk of Noninvasive Ventilation Failure: A Matched Cohort Study With Historical Control” by Drs. Ranieri and Nava shows positive results using ECCO2R to avoid intubation and mechanical ventilation in COPD patients failing NIV.
